A minimally invasive radiofrequency procedure that delivers thermal energy beneath the skin through a small cannula. Tightens the dermis from the underside — appropriate for mid-face, lower face, neck, jawline, and arms.

What it is

DermiTight is a minimally invasive radiofrequency procedure. A small cannula — about the diameter of a thick needle — is inserted through a tiny entry point and passed in a controlled pattern beneath the skin. RF energy is delivered from the tip directly into the underside of the dermis and the fibroseptal network that supports it.

This matters because it changes what the procedure can do. Surface RF and microneedling RF heat the skin from above, with thermal energy traveling downward into the dermis. DermiTight delivers energy where the supporting structure actually lives — at the dermal-subcutaneous junction. The contraction is more direct, the collagen response is more substantial, and the result is more durable than non-invasive RF for the same indications.

It sits in the gap between non-invasive tightening and surgery. Patients with mild-to-moderate laxity who are not yet candidates for a facelift, and who want more than a non-invasive device can deliver, are the right population.

How Dr. Brown approaches it

DermiTight at Esvie is recommended when the indication is laxity that genuinely requires subdermal work — typically the jawline, submental area, lower face, or upper arms. It is not used as a default tightening tool. For mild surface laxity, microneedling RF is less invasive and produces sufficient response for many patients.

The procedure is performed under local anesthesia. Small entry points are placed in inconspicuous locations. The cannula pattern, energy level, and number of passes are calibrated to the anatomic area and the degree of laxity. Real-time temperature monitoring is part of the safety protocol — the goal is therapeutic dermal heating without thermal injury to the overlying skin or underlying structures.

Most patients see meaningful change from a single treatment. Some areas — particularly the neck and arms — benefit from a second session at three to six months. Compression garments are used after treatment in certain areas to support the contraction phase.

If a patient’s laxity is severe enough that a surgical lift would deliver a categorically better result, Dr. Brown will say so. DermiTight has a ceiling, and pretending otherwise produces disappointed patients and bad outcomes.

What to expect

Day of treatment: Local anesthesia is used; the procedure itself is well tolerated. Plan to be at the office for approximately two hours including preparation. Drive yourself home; mild sedation is not standard.

Days 1–3: Swelling peaks in the first 48–72 hours. Mild bruising at entry points and along the treatment field is common. Compression garments are worn as directed.

Days 4–7: Swelling begins to settle. Most patients return to normal activity, though strenuous exercise is held for one week.

Weeks 2–4: Initial tightening becomes visible as residual swelling resolves.

Months 3–6: Full collagen response matures. Tightening continues to develop through month six. Results are typically durable for two to five years depending on age, skin quality, and lifestyle factors.

Candidacy

Good candidates have mild-to-moderate laxity of the lower face, jawline, neck, submental area, or upper arms — and want significant change without committing to surgery. Patients in their mid-thirties through mid-sixties make up the typical population.

Not a candidate with severe skin laxity that genuinely requires surgical lifting — Dr. Brown will say so plainly and refer when appropriate. Not a candidate with active acne or skin infection in the treatment field. Not a candidate with implanted electrical devices in the treatment area. Not a candidate with bleeding disorders or current anticoagulant use that cannot be safely paused. Not a candidate if pregnant or nursing.

If your concern is surface texture, fine lines, or pore size without true laxity, DermiTight is more procedure than you need. Microneedling RF or a chemical peel series is the better starting point, and Dr. Brown will recommend accordingly.
